ENROLLMENT
This agreement applies to all of my dog(s) enrolled at Ridgewood Canine Camp, Inc. I certify that I am the legal owner of my dog(s); that my dog(s) is currently and properly licensed; that my dog(s) is not aggressive.
I understand that my dog must enter and exit the Ridgewood Canine Camp, Inc. building on a leash.
The information provided on the application is accurate and I agree to notify Ridgewood Canine Camp, Inc. of any changes to my personal or my dog’s information.
VETERINARY RECORDS AND HEALTH REQUIREMENTS
My dog(s) is in good health and has not been exposed to any communicable diseases, including but not limited to parasites, within a 30-day period prior to check-in. I agree to immediately notify Ridgewood Canine Camp, Inc. of any known exposure of my dog to a communicable disease, including but not limited to parasites, and agree to hold my dog out of attending Ridgewood Canine Camp, Inc. until my dog is symptom free for a minimum of 30 days or with written veterinary clearance.
I agree to maintain current vaccinations and furnish veterinary records to Ridgewood Canine Camp, Inc. as required by Ridgewood Canine Camp, Inc. policy. These records must include proof of vaccinations and/or treatment for:
Rabies, DHLPP (distemper, hepatitis, leptospirosis, parainfluenza, and parvovirus), and Bordetella (kennel cough)
I agree to keep my dog on a year-round heartworm and flea and tick control program.
I understand that it is required that my dog be spayed/neutered (after age 6 months) to enroll at Ridgewood Canine Camp, Inc.

I agree that Ridgewood Canine Camp, Inc. can contact my dog’s veterinarian for their health information.
I understand that it is required that my dog is on a flea and tick control program. If my dog has fleas and/or ticks at time of check-in, they will not be admitted into the daycare and will not be admitted in the future until symptom free. If any fleas and/or ticks are found on my dog after check-in, I authorize Ridgewood Canine Camp, Inc. to treat my dog as deemed necessary by a flea and tick application. I understand that I will be contacted prior to any treatment, and I am responsible for the cost of such services.
I understand that if Ridgewood Canine Camp, Inc. deems that my dog’s nails are too long and may be a danger to either my dog or other dogs, Ridgewood Canine Camp, Inc. will cut my dog’s nails without prior notification and at my expense.
MEDICATIONS
I understand that Ridgewood Canine Camp, Inc. will only administer oral or topical medications. I will provide such medications with prescription from veterinarian and in the original container from the veterinarian.
AUTHORIZATION OF MEDICAL CARE
If my dog is ill or injured while participating in activities at Ridgewood Canine Camp, Inc., Ridgewood Canine Camp, Inc. will make every reasonable effort to reach me pursuant to the contact information I have provided Ridgewood Canine Camp, Inc. However, if Ridgewood Canine Camp, Inc. is unable to reach me, I consent to Ridgewood Canine Camp, Inc. seeking appropriate veterinary care. I further understand, that I am responsible for any veterinary expenses.
VICIOUS TENDENCIES
I am not aware of any vicious tendencies by my dog.
PHOTOGRAPHS AND STATEMENTS
I agree that my dog may be videotaped, photographed and/or recorded. Ridgewood Canine Camp, Inc. shall retain the exclusive rights to the results and all proceeds of such tapings, photographs and recordings with the rights to use in any manner.
RIGHT TO DECLINE
I understand that Ridgewood Canine Camp, Inc. reserves the exclusive right to decline participation or to terminate participation in activities at Ridgewood Canine Camp, Inc. by my dog at any time for any reason.
CANCELLATIONS
I understand that failure to cancel daycare reservations 24 hours in advance will result in the charge of a full priced day.
I understand that failure to cancel boarding reservations 48 hours in advance will result in the charge of one full priced day.
ABANDONMENT
If my dog is not picked up by the end of the business day or scheduled pick-up time, I authorize Ridgewood Canine Camp, Inc. to take whatever action it deems necessary for the continuing care of my dog. I further agree to pay the cost of such care as provided by Ridgewood Canine Camp, Inc. upon demand, in addition to other fees already incurred for services provided by Ridgewood Canine Camp, Inc. I understand that the hours of operation are Monday through Friday from 7:00am to 6:30pm, Saturday and Sunday 8:00am-9:00am and 5:00pm-6:00pm. Dogs picked up after hours will be charged a late fee of $1per minute.
I agree that if my dog is not picked up within 24 hours of the scheduled pick-up date and time, the dog will be considered abandoned. I accept responsibility of any expenses incurred for the transport, overnight care, and/or placement or adoption of said dog, in addition to other fees already incurred for services provided by Ridgewood Canine Camp, Inc.
AGREEMENT TO PAY
Ridgewood Canine Camp, Inc. accepts check, cash, or select credit cards. I agree to pay the service rates in effect for my dog’s participation in activities at Ridgewood Canine Camp, Inc. I further agree to pay for any additional services requested. All services must be paid in full before my dog will be released. I understand that rates are subject to change without notice.
Prepaid packages are non-refundable. I understand that I will be charged a $25 fee for returned checks.
I understand that if I do not meet my obligations under this agreement, my account may be turned over to a collection agency, and that I will be liable for collection costs and all reasonable attorneys’ fees and court costs (including, without limitation, reasonable expert witness fees) in addition to any unpaid balance.
I understand that Ridgewood Canine Camp, Inc. will make every effort to return items left with my dog. However these items may not be returned or they may be returned in poor condition. Ridgewood Canine Camp, Inc. is not responsible for said items.
LIABILITY WAIVER
I AGREE AND UNDERSTAND THAT THERE ARE INHERENT RISKS WHEN DEALING WITH ANIMALS AND HUMANS IN GROUP SITUATIONS. SUCH RISKS INCLUDE PROBLEMS ASSOCIATED WITH ROUGH PLAY, INCLUDING BUT NOT LIMITED TO:
- EXPOSURE TO PARASITES, VIRUSES, AND OTHER MEDICAL CONDITIONS PASSED FROM DOG-TO-DOG OR
- PERSON-TO-DOG;
- AGGRAVATION OF EXISTING MEDICAL CONDITIONS;
- SPRAINS, STRAINS, BITES, BROKEN BONES;
- FATIGUE, DEHYDRATION, BLOAT, NICKS, CUTS OR DEATH;
- DEVELOPMENT OF KENNEL COUGH EVEN IF MY DOG IS VACCINATED FOR BORDETELLA.
I FURTHER UNDERSTAND THAT NOT EACH AND EVERY POTENTIAL RISK CAN BE LISTED ABOVE BUT, NONETHELESS I AGREE THAT THE BENEFITS ASSOCIATED WITH DOG SOCIALIZATION OUTWEIGH THE POSSIBLE RISKS, THEREFORE, I HEREBY VOLUNTARILY RELEASE, FOREVER DISCHARGE, AND AGREE TO HOLD HARMLESS AND INDEMNIFY RIDGEWOOD CANINE CAMP, INC., ITS SHAREHOLDERS, DIRECTORS, EMPLOYEES, STAFF, AGENTS, CONTRACTORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL LIABILITY, CLAIMS, DEMANDS, ACTIONS, OR RIGHTS OF ACTION, WHICH ARE RELATED TO, ARISE OUT OF, OR ARE IN ANY WAY CONNECTED WITH MY DOG’S PARTICIPATION IN ACTIVITIES AT RIDGEWOOD CANINE CAMP, INC., OR WHILE MY DOG IS IN THE CARE OF RIDGEWOOD CANINE CAMP, INC., INCLUDING THOSE ALLEGEDLY ATTRIBUTABLE TO THE NEGLIGENT ACTS OR OMISSIONS OF RIDGEWOOD CANINE CAMP, INC. OR ITS STAFF OR AGENTS.
FURTHER, I UNDERSTAND THAT I MAY BE EXPOSED TO CERTAIN RISKS WHEN BRINGING MY DOG TO PARTICIPATE IN ACTIVITIES AT RIDGEWOOD CANINE CAMP, INC. OR WHEN PICKING UP MY DOG FROM PARTICIPATING IN ACTIVITIES AT RIDGEWOOD CANINE CAMP, INC. SUCH RISKS MAY INCLUDE PROPERTY DAMAGE AND/OR PHYSICAL INJURY INSIDE OR OUTSIDE THE FACILITY, SUCH AS FROM FALLING, SLIPPING, ILLNESS, AND/OR DOG BITES. THEREFORE, I HEREBY VOLUNTARILY RELEASE, FOREVER DISCHARGE, AND AGREE TO HOLD HARMLESS AND INDEMNIFY RIDGEWOOD CANINE CAMP, INC., ITS SHAREHOLDERS, DIRECTORS, EMPLOYEES, STAFF, AGENTS, CONTRACTORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL LIABILITY, CLAIMS, DEMANDS, ACTIONS, OR RIGHTS OF ACTION, WHICH ARE RELATED TO, ARISE OUT OF, OR ARE IN ANY WAY CONNECTED WITH MY DOG’S PARTICIPATION IN ACTIVITIES AT RIDGEWOOD CANINE CAMP, INC., OR WHILE MY DOG IS IN THE CARE OF RIDGEWOOD CANINE CAMP, INC., OR WHILE MY DOG OR I OR THOSE INDIVIDUALS THAT MY BE PRESENT WITH ME, ARE ON THE PREMISES OCCUPIED BY RIDGEWOOD CANINE CAMP, INC., INCLUDING THOSE ALLEGEDLY ATTRIBUTABLE TO THE NEGLIGENT ACTS OR OMISSIONS OF RIDGEWOOD CANINE CAMP, INC. OR ITS STAFF OR AGENTS.
FURTHER, I RELEASE RIDGEWOOD CANINE CAMP, INC. AND WAIVE ALL CLAIMS AND LIABILITY AGAINST RIDGEWOOD CANINE CAMP, INC. FOR, DAMAGE TO, OR LOSS OF, PERSONAL EQUIPMENT OR BELONGINGS PROVIDED BY ME FOR MY DOG WHILE MY DOG IS IN THE CARE OF RIDGEWOOD CANINE CAMP, INC.
ATTORNEY FEES, APPLICABLE LAW & VENUE
SHOULD RIDGEWOOD CANINE CAMP, INC., OR ANYONE ACTING ON THEIR BEHALF, BE REQUIRED FOR ANY REASON TO INCUR ATTORNEY FEES AND COSTS TO ENFORCE OR DEFEND THIS AGREEMENT, I AGREE TO INDEMNIFY AND REIMBURSE RIDGEWOOD CANINE CAMP, INC. FOR SUCH FEES AND COSTS. FURTHER I AGREE AND UNDERSTAND THAT ANY DISPUTES ARISING OUT OF THIS AGREEMENT WILL BE DECIDED PURSUANT TO THE LAWS OF THE COMMONWEALTH OF PENNSYLVANIA AND VENUE SHALL BE IN BERKS COUNTY.
VALID DATES
THIS AGREEMENT, WAIVERS AND AUTHORIZATIONS WILL REMAIN VALID AND IN FORCE AS LONG AS AND WHENEVER MY DOG PARTICIPATES IN ANY ACTIVITY AT OR WITH RIDGEWOOD CANINE CAMP, INC.
WARNING
BY SIGNING THIS DOCUMENT, I ACKNOWLEDGE THAT IF MY DOG IS OR I AM HURT OR PROPERTY IS DAMAGED DURING MY DOG’S PARTICIPATION IN ACTIVITIES AT RIDGEWOOD CANINE CAMP, INC., I MAY BE FOUND, BY A COURT OF LAW, TO HAVE WAIVED MY RIGHT TO MAINTAIN A LAWSUIT AGAINST RIDGEWOOD CANINE CAMP, INC. ON THE BASIS OF ANY CLAIM FROM WHICH I HAVE RELEASED THEM HEREIN. I HAVE HAD SUFFICIENT OPPORTUNITY TO READ AND FULLY UNDERSTAND THIS ENTIRE DOCUMENT AND I AGREE TO BE LEGALLY BOUND BY ITS TERMS.
